Job summary

Avera at Home Pierre is seeking a compassionate Certified Nurse Aide to provide personal care and daily living assistance to patients in home or facility settings. You will work with a multidisciplinary team and follow established care plans to support the health and comfort of elderly or disabled individuals.

The role requires CNA certification, valid driver’s license, and the ability to document care accurately.

Qualifications

• Must have a valid CNA certification and driver’s license at hire.
• Strong communication and patient-care skills.
• Ability to document care accurately and maintain patient confidentiality.

Responsibilities

• Provide personal care and assistance with activities of daily living.
• Perform housekeeping, laundry, meal preparation, and shopping as needed.
• Assist patients with social activities per care plan and coordinate with nursing staff.
• Maintain timely, accurate documentation and report changes in patient status.
• Ensure safety and adherence to infection control practices and policies.

A Brief Overview

Provides personal care for elderly, convalescent or disabled persons in home or facility settings. Demonstrates the knowledge and skills necessary to provide care for the population served by Avera.

What You Will Do

• Provides assistance to patients with their activities of daily living.
• Provides personal care for patients as directed and delegated by the nurse, according to the established plan of care.
• Provides services such as housekeeping, laundry, meal preparation, and shopping.
• Assists patient with social activities as prescribed by the plan of care.
• Maintains timely and accurate documentation.
• Reports changes in patient status to supervisor.
• Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patient served in the agency.
• Uses equipment and supplies effectively and efficiently.
• Adheres to environmental, safety and infection policies and procedures.

Required Education, License/Certification, Or Work Experience

• Drivers License - Licensing Board Upon Hire
• Certified Nurse Aide (CNA) - Licensing Board If working in the state of Minnesota Upon Hire

Preferred Education, License/Certification, Or Work Experience

• High School or GED Equivalent
• Certified Nurse Aide (CNA) - Licensing Board An active license in the state of practice
• Less than 1 year in home health care or related health care field
