Qualifications

• LPN with home health intake experience required.

Responsibilities

• Serve as liaison with referral sources and health professionals.
• Receive and coordinate all incoming calls and provide basic information to access services.
• Communicate with patients and families about initiation of care/services.
• Complete intake screening and preliminary admission decisions.
• Receive and log referrals and prepare reports for inquiries.
• Promote care/services to prospective patients/clients.
• Support field staff with scheduling and appointments.
• Adhere to standards of practice and regulations; handle PHI per law.
• Enter patient data into the MIS and create patient files.
• Assist with hiring and maintain employee files.
• Engage community to increase visibility and drive growth.
• Work as part of a high-performance team.
