Job summary

Connecticut-Community-Care is seeking an organized care coordination support to assist in coordinating person-centered services in the community, monitoring plans, and helping clients access long-term care.

Responsibilities include scheduling assessments, documenting in CONNEXUS, assisting with applications, maintaining records, and collaborating on an interdisciplinary team to promote CCC services.

Qualifications

• High School Diploma required; Associate Degree in Health or Human Service preferred.
• Project management and organizational skills.
• Strong verbal and written communication skills.
• Ability to work with clients, families, providers and other staff.
• Knowledge of community resources preferred.
• Ability to maintain confidentiality of consumer and company information.
• Ability to understand and articulate multiple programs.
• Familiarity with the needs of a geriatric population is preferred.
• Working knowledge of Microsoft Office products.

Responsibilities

• Coordinate person-centered services for the community with monitoring via phone or home visits.
• Assist Care Managers with scheduling assessments, placement, and care orders; update medications and diagnoses in CONNEXUS.
• Complete applications and required documentation; manage entitlement and transportation applications.
• Triages client, family and provider contacts with appropriate follow-up.
• Support clerical staff and participate in interdisciplinary team activities.
• Promote CCC services and assist in staff mentoring as needed.
• Perform related duties as assigned.
