Qualifications

Responsibilities

• Assess Cardiothoracic patients' clinical and psychosocial needs according to standards of practice.
• Develop, implement and evaluate care plans with patients and families.
• Collaborate with the interdisciplinary team to coordinate care.
• Facilitate safe patient transitions and enhance perioperative experiences.
• Lead staff education, equipment/supply needs, and physician preferences.
• Ensure accurate documentation and data submission per regulatory requirements.
