Job summary

Mercy Medical Center in Baltimore is seeking a Cardiology & Heart Failure Clinical Triage Registered Nurse to triage symptoms, educate patients, and coordinate care within our cardiology and heart failure teams.

The RN will manage clinical messages, provide education, and ensure timely escalation of concerns to the appropriate clinician. Join a dedicated team focused on improving access to care and patient outcomes.

Qualifications

• Active RN License
• BLS/CPR Certification required upon hire
• Minimum 1 year of current clinical experience
• Effective communication with patients and staff
• Team player in a multidisciplinary environment

Responsibilities

• Review and triage Epic In Basket messages, patient portal messages, and clinical requests.
• Respond to patient questions within RN scope and protocols.
• Assess cardiovascular symptoms and triage for escalation when needed.
• Identify patients needing urgent evaluation and escalate to clinicians.
• Provide heart failure follow-up and monitor weight, edema, and symptoms.
• Support GDMT optimization with labs, med follow-up, and side-effect assessment.
• Handle medication refill requests and lab/test result questions per protocol.
• Educate patients and caregivers on cardiovascular conditions and care plans.
• Coordinate follow-up appointments, labs, and referrals as needed.
• Assist with post-discharge follow-up to reduce readmissions.
• Identify barriers to care and arrange referrals/resources.
• Document communications and assessments in EMR.
• Contribute to quality improvement in cardiovascular care.
