Job summary

Lee Health in Fort Myers, FL, is seeking a Pharmacist to join the Advanced Heart Failure Center. In this role, you'll work with a multidisciplinary heart failure team to optimize therapy for patients with complex cardiac conditions, including inotrope therapy and LVAD/heart transplant care.

Qualifications

• Pharmacists must hold a PharmD or equivalent BS Pharmacy with eligible experience.
• Experience with heart failure therapies and inpatient hospital settings is preferred.
• Strong communication and analytical skills are required.

Responsibilities

• Collaborative Practice Agreement (CPA)-based medication management, including initiation, titration, and monitoring of therapy.
• Optimize heart failure guideline-directed medical therapy (GDMT).
• Medication reconciliation and transitions-of-care support to reduce readmissions.
• Patient education and counseling on medications, adherence, and side-effect management.
• Monitoring labs, vital signs, and clinical response to therapy.
• Active participation in clinic visits, care coordination, and EMR documentation.
• Comprehensive MTM.
• Direct collaboration with specialty pharmacy services to ensure continuity of high-cost therapies.
• Support with prior authorizations and medication access.
