Qualifications

Responsibilities
- Coordinate inpatient and ambulatory care for heart failure patients.
- Collaborate with multidisciplinary teams.
- Participate in Shock Team for inpatient consultative responsibilities.

BC/BE by the ABIM in Advanced Heart Failure/Transplant
Management of LVAD, heart transplant
Excellent interpersonal skills

Education
Completion of ACGME accredited heart failure fellowship
