Job summary

Tryon Medical Partners is seeking a Patient Care Coordinator to ensure the physician’s plan of care is accurately completed and to coordinate referrals with physicians, hospitals and community services. You will schedule follow-up visits and assist patients with questions related to delivery system access.

This full-time role supports the Cardiology team at SouthPark, Monday to Friday 6:30 am to 3:30 pm, and requires prior healthcare office experience and solid EHR skills.

Qualifications

• Minimum one year in a medical office or healthcare setting.
• Experience with EHR and scheduling systems.
• Strong patient communication skills.

Responsibilities

• Schedules ancillary appointments and referrals.
• Follows up on referrals and appointments.
• Schedules physician return appointments.
• Completes forms to execute physician orders.
• Obtains and sends patient medical records.
• Ensures records are sent within one work day.
• Obtains ancillary results and referring notes promptly.
• Verifies insurance coverage and patient demographics.
• Updates charts and coordinates with care team.
• Work at multiple Tryon office locations.
• Other duties as assigned.
