Qualifications

• Graduate of an accredited nursing program.
• Completed an accredited Nurse Practitioner program.
• Current New Hampshire APRN license.
• Current DEA registration.
• Able to obtain professional liability insurability through MCH.

Responsabilités

• Perform comprehensive cardiological assessments and patient histories.
• Diagnose and manage a broad range of cardiac conditions.
• Order, interpret labs, diagnostics and imaging.
• Develop individualized, evidence-based treatment plans.
• Prescribe medications per state guidelines.
• Collaborate with physicians and healthcare teams to optimize outcomes.
• Monitor patients between visits and ensure continuity of care.
• Triage calls and provide clinical consultations as needed.
• Educate patients and families on prevention, treatment and lifestyle changes.
• Promote heart health through community outreach and employer wellness.
• Maintain accurate clinical documentation and participate in quality improvement.
