Job summary

Monadnock Community Hospital's Jaffrey Family Medicine clinic seeks a compassionate Nurse Practitioner to join our primary care team. This on-site, full-time position offers independence with collaboration from experienced physicians and a strong commitment to community health.

The NP will perform histories, exams, diagnose and manage conditions, develop treatment plans, and provide patient education. Licensure, DEA, and liability coverage eligibility are required.

Qualifications

• Graduate of an accredited School of Nursing.
• Successful completion of an approved Nurse Practitioner program.
• Current New Hampshire Advanced Registered Nurse Practitioner (ARNP) license.
• Current DEA registration.
• Eligibility for professional liability insurance coverage through MCH.
• Preferred: One or more years of experience in a clinical or ambulatory practice setting.

Responsibilities

• Conducting comprehensive patient histories and physical examinations.
• Assessing, diagnosing, and managing acute and chronic conditions.
• Developing individualized treatment plans and coordinating patient care.
• Ordering and interpreting laboratory tests and diagnostic studies.
• Prescribing treatments and monitoring patient progress.
• Providing preventive care and patient health education.
• Collaborating with physicians and other healthcare providers to ensure high-quality, coordinated care.
• Making referrals to specialists and community resources.
• Triaging patient calls and providing clinical consultation when appropriate.
