Job summary

Qualifications

• Master's degree in nursing for acute/primary care.
• Progressive clinical experience relevant to NP practice.
• Professional certification by a relevant organization preferred.
• Licensure: New York State RN with current registration.
• State Certification: NP certification by NYS Education Department or equivalent.
• Collaborative practice agreement with a physician and designated protocols filed with NYS Ed.
• BLS/CPR certification as standard requirement.

Responsibilities

• Develops and implements a collaborative practice agreement and reviews it periodically.
• Assesses health status through history and physical examinations.
• Initiates therapeutic and diagnostic procedures; prescribes and adjusts medications.
• Provides teaching on health promotion, disease prevention, and chronic illness management.
• Documents findings and plan of care in the medical record.
• Consults with physicians per the collaborative agreement.
• Initiates referrals to specialty services for continuity of care.
• Participates in team meetings to support interdisciplinary care.
