Responsibilities

• Develops and implements collaborative practice agreement and reviews it periodically.
• Assesses health status via history and exam.
• Initiates therapies, diagnostics, and labs per protocols.
• Provides health promotion, disease prevention, and care management education.
• Documents findings and plans in medical records.
• Consults with physician per agreement.
• Initiates referrals to specialty services and supports continuity of care.
• Participates in team meetings to foster interdisciplinary care.
• Reviews patient records with collaborating physician at least every three months.

Education

Master's degree in nursing / NP program

NYS RN license with current registration

Licensed NP in New York

Certification by NY/state NP authority
