Job summary

Grand Itasca Clinic & Hospital, part of Fairview Health Services, seeks an enthusiastic LPN/MA for our Cardiology clinic. The role involves patient flow, data collection, med administration, EMR updates, and collaboration with providers to deliver high-quality care.

The position emphasizes teamwork, patient advocacy, and adherence to nursing standards. Experience in ambulatory care is preferred, with a strong commitment to attendance and a positive patient experience.

Qualifications

• Current LPN Minnesota state license and certified/registered Medical Assistant.
• Ambulatory health care setting experience preferred.
• Ability to work efficiently, accurately, and maintain positive working relationships.
• BLS certification to be completed within two months of employment.
• Follow Grand Itasca Clinic and Hospital policies, vision, mission and values.
• Maintains reliable and consistent attendance.

Responsibilities

• Conduct focused assessment and report changes to licensed health care provider.
• Collaborate with health care team to develop/modify patient health plans of care.
• Implement patient care interventions ordered by licensed health care provider.
• Assign nursing activities to other LPNs.
• Assign and monitor tasks delegated to CNAs or other unlicensed personnel.
• Maintain patient care quality, advocacy, and effective communication with the team.
• Provide health information to patients and review education materials.
• Contribute input for policies and procedures.
• Adhere to the Minnesota Nurse Practice Act and demonstrate competency within scope of practice.
