Endeavor Health’s Center for Cardiac Health – Heart Failure seeks an Advanced Practice Provider (APN/PA) to deliver autonomous, patient-centered care for high-acuity cardiac patients at Edward Hospital in Naperville, IL. The role is outpatient-oriented with a Monday–Friday schedule and potential travel to Elmhurst.

Qualified applicants will have IL APN/PA licensure, DEA and BLS/CPR certs, a master’s degree in a relevant specialty, and prior cardiology experience.

Qualifications

• Active IL Advanced Practice Licensure (APN) or PA licensure required.
• Federal DEA and Illinois Controlled Substances License required.
• Master’s level program in specialty area completed.
• BLS/CPR certification required.

Responsibilities

• Deliver patient-centered care to high-acuity cardiac patients under physician supervision.
• Order and interpret diagnostic tests and develop differential diagnoses.
• Prescribe evidence-based pharmacological and nonpharmacological treatments.
• Coordinate care with multidisciplinary teams and ensure post-discharge follow-up.

Job Description

Exciting opportunity to Join Endeavor Health’s Center for Cardiac Health–Heart Failure team in an autonomous role delivering patient-centered care to high-acuity patients under the direction of a collaborating physician! Key responsibilities include:

• Obtaining comprehensive patient histories and performing physical examinations
• Ordering and interpreting diagnostic tests
• Developing differential diagnoses and prescribing evidence-based pharmacological and nonpharmacological treatments
• Managing acute and chronic conditions
• Providing ongoing education to patients and their families
• Coordinating care with physicians and interdisciplinary teams to optimize outcomes and support post-discharge follow-up and transitional care
• Supporting quality-improvement and patient-safety initiatives
• Contributing to the development and implementation of organizational and service-line goals
• Adhering to all Endeavor Health policies, procedures, and standards.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
• Education: Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Prior APN/PA Cardiology experience required

Special Knowledge And Skills Required

• APP experience in cardiology required

Preferred Qualifications

• Experience working with Heart Failure patients preferred
