Endeavor Health's Center for Cardiac Health—Heart Failure in Naperville, IL is seeking an Advanced Practice Provider (APN/PA) to deliver autonomous, patient-centered care for high-acuity patients under physician direction.

This full-time outpatient role involves taking comprehensive histories, performing exams, ordering and interpreting tests, developing differential diagnoses, and prescribing evidence-based treatments.

Qualifications

• Cardiology APN/PA experience required.
• IL APN/PA licensure with DEA and Illinois CDS license required.
• Master's degree in nursing (APN/PA) or equivalent.

Responsibilities

• Obtain comprehensive patient histories and perform physical examinations.
• Order and interpret diagnostic tests.
• Develop differential diagnoses and prescribe evidence-based treatments.
• Manage acute and chronic conditions.
• Provide ongoing education to patients and families.
• Coordinate care with physicians and interdisciplinary teams to optimize outcomes and support post-discharge follow-up and transitional care.
• Support quality-improvement and patient-safety initiatives.
• Contribute to the development and implementation of organizational and service-line goals.
• Adhere to Endeavor Health policies, procedures, and standards.

Hourly Pay Range:

$57.79 - $79.76 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

• Position: Advanced Practice Provider (APN/PA) - Center for Cardiac Health - Heart Failure
• Location: Edward Hospital, Naperville, IL
• Full Time/Part Time: Full-Time (average of 40 hours/week)
• Hours: Monday - Friday between 7:00AM - 5:30PM. Call in the future may be required.
• Required Travel: Possible travel to Elmhurst Specialty Clinic may be required.

Job Description:

• Managing acute and chronic conditions
• Providing ongoing education to patients and their families
• Coordinating care with physicians and interdisciplinary teams to optimize outcomes and support post-discharge follow-up and transitional care
• Supporting quality-improvement and patient-safety initiatives
• Contributing to the development and implementation of organizational and service-line goals
• Adhering to all Endeavor Health policies, procedures, and standards.

Qualifications:

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required

• Education: Completion of master's level program in specialty area
• Certification: BLS/CPR certification required

• Experience: Prior APN/PA Cardiology experience required

Special Knowledge and Skills Required:

• APP experience in cardiology required

Preferred Qualifications:

• Experience working with Heart Failure patients preferred
