Job summary

University of Florida Jacksonville Physicians, Inc. seeks an Advanced Practice Provider III to deliver direct patient care in a subspecialty outpatient and/or inpatient setting.

You will collaborate with physicians, pharmacists, and other health professionals to manage high-acuity cases and to develop comprehensive care plans. Eligible candidates hold an APRN or PA master’s degree with licensure in Florida and/or Georgia, BLS/ACLS, and preferably PALS.

Qualifications

• 2 years Outpatient/Inpatient setting preferred.

Responsibilities

• Provides timely and appropriate referrals to patients and coordinates care, including scheduling procedures and discharge planning.
• Performs or assists with high-complexity procedures in clinic or inpatient unit and manages devices.
• Develops comprehensive health management plans for high-acuity patients in collaboration with physicians and pharmacists.
• Documents clinical information in the EMR accurately and in a timely manner per billing guidelines.
• May provide education to residents and students and travel up to 25% as required.
