Job summary

Carle Health seeks a Licensed Practical Nurse (LPN) to support RNs with high-acuity patients, perform med administration (including IV piggybacks where appropriate), assist with dressing changes, and document assessments and care plans after the RN completes the initial evaluation. You will collaborate with physicians and APPs, reinforce discharge instructions, and participate in ongoing patient education.

Must hold an active LPN license (IDFPR) and BLS certification.

Qualifications

• Active Licensed Practical Nurse (LPN) license (IDFPR)
• Basic Life Support (BLS) certification within 30 days from hire

Responsibilities

• Passes medications and administers IV piggybacks and fluids as prescribed
• Charts assessments on assigned patients after RN initial evaluation
• Charts and coordinates care plans with the RN-initiated plans
• Communicates with physicians/APPs to receive orders and coordinate care
• Performs dressing changes and trach care as needed
• Inserts and cares for NG tube and feedings, and changes ostomy as required
• Reinforces discharge instructions and educates patients and families
