Job summary

Carle Health is seeking an LPN to support high-acuity patients alongside RNs. The role involves medication administration, IV piggybacks, NG tube care, trach care, and ostomy management, with charting of assessments and care plans after the RN’s initial evaluation.

Collaboration with physicians and APPs is essential to coordinate orders and patient care. Responsibilities also include dressing changes, discharge education reinforcement, and adherence to care plans as opened by the RN.

Qualifications

• Licensed Practical Nurse (LPN) with current license (IDFPR) required.
• BLS certification within 30 days of hire (AHA) required.
• College Diploma: Licensed Practical Nurse (Required) per state requirements.
• Experience with med administration and IV piggybacks is desirable.

Responsibilities

• Passes all medications and hangs IV piggybacks and fluids (excluding IV Push).
• Charts assessments on assigned patients after RN initial assessment.
• Charts care plans opened by the RN; follow-up with ongoing plans.
• Communicates with physicians/APPs to receive orders and coordinate care.
• Complete dressing changes and trach care as needed.
• Insert and care for NG tube and feedings through the NG tube.
• Care for and change ostomy; reinforce discharge instructions.
