Job summary

SSM Health in Missouri is seeking a Cardiac and Pulmonary Rehab professional to provide direct patient care, monitor patients, and guide therapeutic exercise programs.

The role involves collaborating with nurses, physicians and an interdisciplinary team, performing assessments, developing exercise prescriptions, coordinating testing and follow-up, and contributing to quality improvement efforts through education and outreach.

Qualifications

• Bachelor's degree required.
• No experience required.

Responsibilities

• Assesses patient needs daily through chart review, unit nurses or cardiac nurse, physicians and interdisciplinary team.
• Provides exercise prescription for patients utilizing test data, patient status and current medication regimen; updates prescription according to the patient's response.
• Schedules and coordinates patient assessment, testing, results reporting, treatment and follow up as needed.
• Assesses hemodynamic response and patient tolerance to activity progression and utilizes information to provide appropriate exercise prescription.
• Performs appropriate documentation of monitored ambulation, education and discharge planning per departmental policies, guidelines and practices as pertinent for setting.
• Participates in routine maintenance and calibration of the test equipment to ensure its proper operating condition and reporting the need for major repair.
• Assists in recruiting/interviewing and onboarding of new associates.
• Creates quality improvement monitoring strategies and improvement initiatives.
• Assists in monitoring of staff performance.
• Provides exercise information for patients, peers and the community which may include public speaking and education classes.
