Job summary

Qualifications

• Must possess a current, valid RN license in state of practice.
• BLS certification is required.
• Experience in an acute care hospital setting within the last 12 months preferred.

Responsibilities

• Assess patient needs, plan discharge, and coordinate care with the health team.
• Implement the plan of care and supervise care activities as needed.
• Document patient data and care provided in the medical record.
• Evaluate patient progress and adjust care as required.
• Collaborate with physicians, nursing, and support staff to ensure quality care.
