Qualifications

Responsibilities

Assessment/Diagnosis: perform initial and ongoing health status assessments.
Outcomes identification and planning: document planned interventions to meet patient needs.
Implementation/evaluation: monitor response to interventions and adjust plan with the team.
Leadership: participate in process improvement to improve clinical quality and safety.
Operations: support daily unit operations and patient care activities.
