Job summary

Qualifications

• Proven RN license in New York or eligibility.

Responsibilities

• Assess the patient’s condition and nursing needs; coordinate with other disciplines.
• Develop written individualized care plans including discharge planning.
• Administer medications and direct patient care as needed.
• Practice infection control and maintain a clean, safe environment.
• Communicate changes in patient condition to the care team.
• Maintain and update care plans and patient education in medical records.
