Job summary

Yale NewHaven Health is seeking a Registered Nurse to support Cardio-Pulmonary services, performing assessments, planning and coordinating care on a cardiology unit. Collaboration with the healthcare team is essential to optimize patient outcomes.

The role requires CCU/step-down experience, AC/BSL certification, and ability to perform education and discharge planning. A valid CT RN license is required and stress testing experience is a plus.

Qualifications

• Graduate of an accredited BSN program preferred.
• Proficient in computer software and electronic patient records.
• Ability to use internet for literature searches regarding patient care.

Responsibilities

• Assess patient's physical and psychosocial status throughout the continuum of care.
• Develop and document an individualized plan of care involving the patient, family and healthcare team.
• Coordinate and implement the plan of care with the healthcare team.
• Evaluate the effectiveness of the plan and readiness for discharge from care programs.
• Educate patient and family; document outcomes.
• Serve as preceptor to new staff.
• Ensure performance in customer service, teamwork and resource utilization as outlined in reviews.
• Maintain documentation and compliance to support reimbursement and smooth operations.
• Establish and maintain IV access.
