Job summary

Millennium Physician Group is seeking a Primary Care Advanced Practice Provider (APRN/PA) in Florida to deliver comprehensive primary care in collaboration with physicians and an interdisciplinary team.

You will assess, diagnose, and manage acute and chronic conditions, develop care plans, and participate in quality initiatives to improve patient outcomes in a value-based care environment.

Qualifications

• Licensed Florida APRN or PA in good standing.
• National Board Certification as APRN/PA.

Responsibilities

• Provide comprehensive primary care in collaboration with physicians and care team.
• Perform patient assessments, examinations, and care planning.
• Diagnose and treat acute and chronic conditions using evidence-based guidelines.
• Develop care plans including chronic disease management and palliative services within scope.
• Interpret labs/imaging to inform treatment decisions.
• Prescribe medications and order home health and DME as appropriate.
• Identify care gaps and document HCC for value-based care initiatives.
• Educate patients on disease management and preventive care.
• Coordinate care across providers, caregivers, and community resources.
• Ensure compliant documentation in EMR.
