Job summary

Capital Health in Hopewell, NJ seeks a Capacity Management RN for full-time night shift (6:00pm-6:30am with rotating weekends and holidays) to coordinate patient flow, bed management, and the Transfer Center.

You will partner with Patient Access, Emergency and Surgical Services to optimize throughput, mentor staff, and activate surge plans as needed, ensuring timely transfers and discharge planning.

Qualifications

• Associate's degree in nursing required.
• Four years acute care hospital experience including telemetry.
• AHA BLS for Healthcare Providers required.
• Strong verbal and written communication and customer service skills.
• Ability to prioritize in a fast-paced environment and collaborate across teams.

Responsibilities

• Coordinate Patient Logistics and Transfer Center functions with bed flow management.
• Oversee patient flow activities and collaborate with Patient Access, Emergency and Surgical Services.
• Communicate with unit staff on transfers and admissions to facilitate timely movement.
• Collaborate with physicians, care managers, and floor staff for discharge/transfer coordination.
• Assess patient status/acuity to prioritize admissions and transfers.
• Serve as clinical resource for patient placement and minimize wait times.
• Ensure throughput and compliance with transfer processes; delegate tasks as needed.
• Identify barriers to discharge and work on throughput improvements.
• Activate Surge/Capacity plan and monitor bed status and closures.
• Coordinate transport and bed tracking; work with EMS and external transfers.
• Educate care teams on patient flow strategy and maintain effective communication.
• Perform other duties as assigned.
