Job summary

FeldCare Connects is seeking a Cantonese-speaking Occupational Therapist to deliver exceptional home health services in Diamond Bar and surrounding areas. This Independent Contractor role serves adults of all ages, performing assessments, creating treatment plans, and guiding patients to improve daily functioning.

The OT will evaluate outcomes, coordinate with family and care teams, and may be involved in home assessments and adaptive equipment recommendations.

Qualifications

• Ability and enthusiasm in working with all ages.
• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.
• Bilingual is a plus.

Responsibilities

• Assess and evaluate patient care needs related to functional status and activities of daily living.
• Develop and implement treatment plans to improve wellbeing.
• Assess progress and adjust care as needed.
• Coordinate with patients, families, and health care team.
