Job summary

FeldCare Connects in Los Angeles seeks a self-motivated Physical Therapist to deliver high-quality home health care to adults across the area. As an Independent Contractor, you will assess needs, develop a treatment plan, and implement interventions to reduce pain and restore function in patients' homes.

You will monitor progress, document outcomes with accuracy, and communicate clearly with patients and care teams.

Qualifications

• Ability and enthusiasm in working with all ages
• Physical Therapist license and state registration
• Completion of a Physical Therapist curriculum approved by major PT bodies
• Bilingual a plus!

Responsibilities

• Assess and evaluate patient care needs to promote movement, reduce pain, restore function, and prevent disability.
• Determine a treatment plan based on assessment and evaluation, and execute interventions to improve well-being.
• Monitor patient progress and adjust plans as needed; document progress and outcomes with accuracy.
