Job summary

USC Norris Comprehensive Cancer Center in Los Angeles, CA seeks a Healthcare Navigator to provide patient-centered support to cancer patients and families throughout the continuum of care. You will help overcome barriers to treatment, coordinate appointments, and connect patients with community resources.

Reporting to Associate Director, you will work with an interdisciplinary team, educate patients about care plans and rights, document interactions, and advocate for culturally competent care

Qualifications

• Bachelor’s degree in a healthcare, public health, or related field with relevant experience.

Responsibilities

• Serve as primary contact for cancer patients and caregivers throughout treatment and follow-up.
• Assist patients in understanding treatment options, medical instructions, and available support services.
• Identify and address barriers to care including financial hardship, language access, housing, transportation, and psychosocial needs.
• Provide emotional support and connect patients with counseling and community resources.
• Maintain accurate documentation of patient interactions, referrals, and care coordination.
