Job summary

Stony Brook University is seeking an Oncology Nurse Coordinator to work closely with physicians and nurse navigators to coordinate care for cancer patients undergoing active treatment at Stony Brook Medicine. The role involves establishing care maps, coordinating treatments and appointments, educating patients and families, and ensuring high-quality care across the oncology team.

Required: NYS RN license, 3 years acute nursing with 2 years in oncology; BSN or enrollment in BSN program.

Qualifications

• NYS RN license.
• Three (3) years acute nursing experience, with minimum 2 years in oncology.
• Bachelor’s degree in Nursing or current enrollment in BSN program (proof must be submitted).
• Previous experience coordinating outpatient cancer care (preferred).

Responsibilities

• Serve as liaison for assigned physician practices.
• Formulates and updates the plan of care addressing educational, financial, social work needs.
• Perform reassessments including psychological, physical, environmental and functional needs and make referrals.
• Facilitate communication between attending staff, residents, fellows, nurses and secretarial support staff.
• Coordinate and schedule treatment and clinic visits with clinic staff to ensure timely care.
