Job summary

Pacific Health Group is seeking a Community Health Worker (CHW) for a field-based role within the CalAIM program. Expect ~65% field work and ~35% remote work with county-based assignments. Compensation ranges from $21.00 to $24.00 per hour, non-exempt status and hybrid field-based scheduling.

Responsibilities include direct member engagement, care coordination, health education, and outreach. Travel within the county, documentation in real-time, and collaboration with RNs are required.

Qualifications

• High school diploma or equivalent; associate’s or bachelor’s degree preferred.
• Minimum of 2 years in community health, social services, or related field.
• Strong understanding of community resources and social determinants of health.
• Excellent interpersonal, communication, and engagement skills.
• Proficiency with EHR and basic computer systems.
• Bilingual skills preferred (Spanish or other languages).
• Ability to document in real-time and manage multiple systems.

Responsibilities

• Conduct outreach, assessments, and direct member engagement.
• Travel within the county to meet members and partners.
• Support health education, care navigation, and resource coordination.
• Facilitate group sessions and community activities.
• Document services and interactions in real-time.
• Collaborate with RNs and licensed providers; participate in meetings.
