Qualifications

Responsibilities

• Process intake documents and obtain authorizations from Health Plans.
• Handle referrals and verify timely processing per contract.
• Submit authorizations and reauthorizations via MCP portals.
• Maintain patient charts and upload documents into EMR.
• Provide updates to external care partners on patient status.
• Prepare weekly and monthly CalAim MCP reports.
• Uphold HIPAA confidentiality and agency policies.
