Wheatridge Park Care Center is seeking a C.N.A. Resident Scheduling/Transportation Coordinator to join our team. You will assist in coordinating facility staffing and transportation for residents, ensuring appointments are met and communications flow between nursing staff and transportation services.

The role requires a current C.N.A. license, a valid driver’s license, and a willingness to support transportation needs, including unscheduled emergencies.

Qualifications

• Must hold a current C.N.A. license.
• Must hold a valid driver's license with a good driving record.
• High school diploma or GED preferred.
• CPR/First Aid certification preferred; Driver Safety and Training course preferred.

Responsibilities

• Coordinate resident appointments.
• Transport residents to and from medical appointments, admissions, activities, and shopping using the community vehicle.
• Maintain up-to-date records of resident appointments.
• Coordinate and communicate appointments with nursing department and leadership.
• Collaborate with other transport companies to ensure transportation availability.
• Meet driver position requirements and complete ongoing training and driver checks.
• Read transportation schedule daily and ensure all appointments can be met.
• Be flexible for unscheduled transportation needs in emergencies.
• Adjust schedules to assist residents who cannot access the community and appointments independently.
