Job summary

Amazing Care is seeking a dynamic referral relations manager to grow partnerships with hospitals, physicians, rehabs, and government health agencies in the Denver metro area.

You will educate referral sources on home health services, coordinate with the clinical team, and lead outreach initiatives to boost brand awareness and patient-centered care across Colorado.

Qualifications

• Experience in home health or healthcare preferred.
• Strong relationship-building and communication skills.
• Self-motivated, organized, and accountable.
• Valid driver’s license and reliable transportation.
• Familiarity with performance marketing strategies and KPIs.
• Experience working cross functionally with key stakeholders.
• Ability to analyze data effectively to inform decision-making and optimize efforts.

• Excellent follow-up skills.
• Bachelor's degree or equivalent experience.

Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, rehabs, group homes, government health agencies, school district boards, and other healthcare partners.
• Educate referral sources on home health services including pediatric and adult private duty nursing, skilled nursing visits, CNA services, and therapies for home health and pediatric outpatient therapies.
• Coordinate closely with the clinical team to determine staffing availability and appropriateness of referrals.
• Represent Amazing Care with professionalism, integrity, and warmth.
• Participate in strategic growth initiatives aligned with patient-centered care.
• Attend community events to increase brand awareness.
• Foster a sense of community for patients, employees, and referral sources.
• Develop and enhance brand awareness and build relationships with referral sources for Colorado locations.
• Execute innovative and consistent outreach strategies for private duty nursing and intermittent lines of business.
• Cover the Denver Metro areas.
