Job summary

Tennova Healthcare LaFollette Medical Center is seeking a Social Worker (BSW) to support patients and families, promote psychosocial well-being, and coordinate care across the continuum.

Under supervision of licensed social workers, you will assist with psychosocial assessments, discharge planning, and referrals to community resources, ensuring safe transitions and improved patient outcomes.

Qualifications

• Bachelor's Degree in Social Work (BSW) required.
• 1–2 years of healthcare or social services experience preferred.

Responsibilities

• Conducts psychosocial screenings for patients across age groups.
• Identifies emotional, social, and financial needs of patients and families; connects to support or referrals.
• Supports development and implementation of discharge plans with the care team.
• Coordinates referrals to services such as home health, long-term care, rehabilitation, or behavioral health resources.
• Assists in follow-up planning and coordinates communication with external providers and community agencies.
• Provides emotional support to patients and families facing illness or discharge needs.
• Participates in daily interdisciplinary care team discussions and case reviews.
• Maintains accurate documentation in the medical record per policies and regulatory requirements.
• Connects patients to financial assistance, transportation, counseling, and support services.
