Job summary

Valley Regional Medical Center in Brownsville, TX seeks a Social Worker (BSW) to promote patient-centered care by evaluating psychosocial needs and coordinating post-acute services. You will liaise with the care team and community resources to implement discharge plans for Med/Surg patients.

You will identify discharge barriers and educate patients and families on resources, benefits and coverage, supporting informed decisions and smooth transitions.

Qualifications

• Bachelor's degree required.
• Licensing: LMSW/RCSW-I, or LCSW, or LBSW.

Responsibilities

• Promote patient-centered care by evaluating psychosocial needs and coordinating post-acute services.
• Act as liaison between patient and community resources and post-acute care providers.
• Develop and facilitate discharge plans; reassess and modify plans with the clinical team.
• Educate patients and families on community resources, care criteria, benefits and coverage.
• Coordinate meetings with care teams, patients, families and caregivers; support difficult decisions.
