Job summary

Eastern Idaho Regional Medical Center in Idaho Falls seeks a Social Worker (BSW) to assess psychosocial needs and coordinate post-acute care. The role acts as a liaison between patients and community resources, facilitating timely discharge plans and continuity of care across the Med/Surg units.

The candidate will collaborate with a multidisciplinary team to address barriers to discharge, provide patient education, and coordinate post-acute interventions per the care plan.

Qualifications

• BSW degree and licensure required.
• No travel required.

Responsibilities

• Collaborate with RN case managers, acute care nurses, physicians, insurers, and staff to identify and address patient needs.
• Identify and address discharge barriers—financial, emotional, psychosocial, medical, spiritual.
• Perform thorough assessments and develop discharge plans for high‑risk patients with discharge planning needs.
• Obtain input from RN case manager to ensure discharge plan aligns with patient care plan and adjust as needed.
• Coordinate and facilitate meetings between care team and patients/families to support decisions and care continuity.
