Job summary

Qualifications

• BSN required; Illinois RN license or eligibility to licensure is expected.
• Current Basic Life Support (BLS) certification is required.

Responsibilities

• Performs a comprehensive assessment on all assigned patients with documentation.
• Communicates changes in patient’s condition promptly to the care team.
• Uses nursing diagnoses and assessments to set measurable care goals.
• Collaborates with patient, family, and health care team when planning care.
• Assesses discharge needs and makes referrals including palliative care.
• Provides complete discharge instructions and medication reconciliation.
