Job summary

Qualifications

• BSN required and RN license in IL.
• Basic Life Support (BLS) required.
• Strong interpersonal skills and collaboration abilities.
• Analytical thinking and evidenced-based practice.
• ADN acceptable with commitment to obtain BSN within 3 years.

Responsibilities

• Performs comprehensive patient assessments with complete documentation.
• Communicates changes in patient condition promptly.
• Collaborates with patient, family and health care team in planning care and referrals.
• Assists with discharge planning and appropriate referrals, including palliative care.
• Ensures medication reconciliation and patient portal and education resources at discharge.
• Engages in unit safety, quality initiatives and evidence-based care practices.
