Job summary

Bond Health is seeking Licensed Practical Nurses for our Brooklyn facility to work closely with the DON and care for residents. You will administer medications and treatments in line with policies.

Responsibilities include ordering and distributing meds and supplies, following care plans, wound care, documenting observations, and maintaining confidentiality in compliance with HIPAA. NY LPN license required; SNF experience preferred.

Qualifications

• Current license as an LPN valid in the State of New York.
• Skilled Nursing experience preferred.

Responsibilities

• Order prescribed /distribute medications, supplies, and equipment
• Follow a designated care plan in accordance with patients needs
• Perform assigned duties including wound care, treatments, and procedures
• Accurately document observations, interventions, and evaluations pertaining to client
• Maintain confidentiality and rights of residents, family members, employees, and facility information in compliance with HIPPA policies
