Job summary

Qualifications

• Graduation from an accredited school of practical nursing or completion of equivalent study in an approved program in professional nursing.
• Minimum one (1) year clinical experience preferred.

Responsibilities

• Demonstrates proficient knowledge in the treatment of patients requiring nursing care and uses diagnostic results to evaluate.
• Applies knowledge of illnesses, injuries, and diseases and uses available resources in performing the nursing process.
• Performs timely, comprehensive, and accurate reevaluations and documentation at regular intervals as defined by the department, when a significant change occurs in the resident's condition, and when otherwise warranted.
• Is considerate of residents' privacy during the evaluation process and contributes to an individualized plan of care with measurable goals.
• Contributes to the plan of care making changes as the patient’s condition indicates; monitors patient outcomes.
• Monitor continued care needs for discharge and coordinates resources to meet post-institutional requirements.
• Collaborate with Care Manager, physician, and other disciplines in planning interventions to meet resident and family needs in a timely manner.
