Responsibilities

• Assess and treat patients with communication and swallowing disorders in a home care setting.
• Develop and implement individualized treatment plans to achieve patient goals.
• Collaborate with patients, families, and healthcare providers to ensure seamless care coordination.
• Conduct frequent assessments and progress monitoring to adjust treatment plans as needed.
• Maintain accurate and detailed records of patient treatments and outcomes.
• Participate in multidisciplinary team meetings to share knowledge and best practices.
• Stay up-to-date with industry developments and advancements in speech therapy techniques and technology.
