Job summary

Homeside Rehab is actively hiring Speech-Language Pathologists (SLPs) to provide individualized, evidence-based therapy in patients’ homes across the Hudson Valley, NY. You’ll help improve communication, cognition, feeding, and safe swallowing while enjoying a flexible schedule and supportive clinical team.

This role requires NYS SLP licensure and at least 1 year of relevant experience; responsibilities include evaluations, treatment planning, documentation, and collaboration with the care team

Qualifications

• Active New York State SLP license required.
• Minimum 1 year of home health, outpatient, EI, or SNF experience preferred.
• Excellent clinical assessment and communication skills.
• Ability to work independently in a home health setting.

Responsibilities

• Perform comprehensive speech-language evaluations and create individualized treatment plans.
• Provide evidence-based treatment for speech, language, articulation, fluency, voice, cognitive-communication, and swallowing when applicable.
• Deliver skilled SLP services in patients' homes enabling functional independence.
• Complete accurate, timely electronic documentation.
• Collaborate with physicians, caregivers, and the interdisciplinary team.
• Educate patients and caregivers on home programs and communication strategies.
• Communicate effectively with office staff about scheduling and progress.
