Job summary

Apex Rehab Group is seeking an experienced Speech Language Pathologist to join our home care team in Westchester County, NY. You will assess patients, design personalized treatment plans, document care, and adjust goals to achieve optimal outcomes.

Responsibilities include delivering therapy at patients' homes, evaluating progress, collaborating with families, and maintaining detailed records. Licensure in NY and strong communication are essential; home-care experience is a plus.

Qualifications

Active NY state license for practicing speech therapy.
Strong communication and interpersonal skills.
Excellent organizational skills.
Strong charting and documentation skills.
Prior home care experience is preferred but not required.

Responsibilities

Provide therapy services to patients in their homes.
Make evaluations and create treatment plans customized for each patient, setting short and long-term goals.
Assist in the implementation of the treatment plan and adjust as needed for successful outcomes.
Maintain comprehensive records of patient needs, care, and outcomes.
Collaborate with patients and families to ensure understanding and provide tools for success.
