Job summary

Qualifications

Bachelor’s degree in Social Work, Human Services, Psychology, or related field.
Prior experience in case management or social services preferred.
Knowledge of Bronx/NYC community resources and social service systems.

Responsibilities

Conduct client intake assessments to identify needs, goals, and eligibility for services.
Develop and implement individualized care plans with clients and support teams.
Connect clients with community resources, social services, and referral networks.
Monitor client progress and adjust service plans as needed to ensure positive outcomes.
Maintain accurate case documentation and client records.
Advocate on behalf of clients to ensure access to housing, healthcare, and social support services.
Collaborate with community partners, service providers, and ministry staff to coordinate care.
