Job summary

Beth Israel Lahey Health, Inc. in Beverly, MA seeks a NP or PA to launch a novel Bronchiectasis Program in outpatient, population-based care. The role combines navigation and direct clinical care in a weekday outpatient setting.

You will leverage NLP screening of radiology reports, coordinate care, educate patients, and document progress in REDCap while collaborating with primary care and Lung Health Team meetings.

Qualifications

• Licensed NP or PA eligible to practice in MA.
• Strong organizational skills with comfort balancing clinical care and outreach.
• Excellent communication with patients and multidisciplinary stakeholders.
• Proficiency with EHR workflows and registry databases such as REDCap.

Responsibilities

• Screen NLP-flagged cases and confirm program eligibility.
• Outreach to primary care teams and patients via phone, letters, and MyChart.
• Coordinate care pathways including PFTs, labs, imaging, and scheduling.
• Educate patients on care plans, timelines, and escalation procedures.
• Document activities in the EHR and REDCap for program tracking.
• Outpatient visits and longitudinal management of bronchiectasis, lung nodules, COPD/emphysema, and ILD/ILA under physician supervision.
