Beth Israel Lahey Health is seeking an innovative NP or PA for an outpatient Pulmonology APP role focused on launching a Bronchiectasis Program. The position blends technology, population health, and clinical care with NLP screening of radiology reports to identify at-risk patients and drive structured workflows.

The role includes 50% patient navigation and 50% direct clinical care, with opportunities for research, mentorship, and future growth within a grant-supported program.

Qualifications

• Licensed NP or PA eligible to practice in MA.
• Strong organizational skills balancing clinical care and outreach.
• Excellent communication with patients and multidisciplinary stakeholders.
• Proficiency with EHR workflows.

Responsibilities

• Screen NLP-flagged cases via chart reviews to confirm eligibility.
• Outreach to primary care teams and patients via multiple channels.
• Coordinate care: diagnostic pathways and pulmonology scheduling.
• Educate patients on care plans, timelines, and escalation.
• Track activities in the EHR and REDCap registry.
• Conduct outpatient visits and longitudinal management for bronchiectasis, COPD, ILD.
• Perform risk stratification and develop evidence-based care plans.
• Participate in multidisciplinary case reviews and Lung Health Team meetings.
