Genesis OBGYN is seeking a Breast Navigator to guide patients through the breast health journey, coordinating screenings, imaging, and treatment options. You will educate patients, advocate for access to resources, and collaborate with radiology, oncology, and primary care teams to ensure seamless care.

Ideal candidates have a nursing-related degree, 3+ years in breast imaging or navigation, and proficiency with EMR systems and care coordination.

Qualifications

• Bachelor's degree in Nursing, Mammography, or related healthcare discipline.
• 3+ years of experience in breast imaging, oncology, women's health, case management, patient navigation, or related clinical specialty.
• Knowledge of breast cancer screening guidelines, diagnostic pathways, treatment options, and survivorship resources.

Responsabilités

• Guide patients through the continuum of breast care, including screening mammography, diagnostic imaging, biopsy procedures, and surgical consultations.
• Assess individual patient needs and identify barriers that may impact access to services.
• Coordinate appointments, diagnostic testing, referrals, consultations, and follow-up services.
• Monitor patient progress throughout the care continuum to ensure completion of screenings and workups.
• Educate patients on breast health, imaging procedures, and treatment options.
• Advocate for patients by facilitating communication among providers and accessing support resources.
• Serve as a liaison between patients, physicians, imaging, pathology, and referring providers.
• Maintain documentation of patient interactions and care plans in the EMR.
