Job summary

Brain Injury Association of Colorado, Inc. is seeking a full-time professional to serve the Western Slope and across Colorado, providing person-centered, trauma-informed support to individuals with brain injuries or their caregivers.

You will coordinate resources, navigate benefits, and maintain client records while collaborating with a team. Remote work is supported; some travel and semi-annual gatherings in Denver are expected.

Qualifications

  • Bachelor’s degree, or equivalent experience, in a related field.
  • Experience navigating systems in a human services, case management, social work, or other applicable setting.
  • Knowledge of community, state, and federal resources for brain injury, disability, or human services specific to the region this position serves.
  • Experience teaching life skills such as: organization, financial management, healthy lifestyle, social skills, job skills, computer and internet usage.
  • Valid driver’s license and access to your own vehicle for travel within the region this position serves.

Responsibilities

  • Program and Service Delivery - Resource Facilitation
  • Support clients of all ages across Colorado, with a focus on the Western Slope, by offering guidance and resource navigation through phone, email, and in-person interactions as needed (serving a consistent case load of eight clients at all times).
  • Support brain injury survivors in troubleshooting challenges, even in cases where they may be uncertain about the resources they require.
  • Refer clients to internal programs and/or community agencies to meet resource needs.
  • Work collaboratively with team members when they encounter challenges in finding resources for clients or determining the appropriate course of action.
  • Answer calls and respond to voicemails on the Brain Injury Association of Colorado’s main line number.
  • Establish connections with external service providers to maintain an up-to-date inventory of resources and engage/participate in meet and greet opportunities.
  • Take part in staff training sessions and meetings to collaborate with the team and enhance professional skills.
  • Program and Service Delivery - Self-management
  • Meet with clients for an average of one hour per week in their home community (serving about four unique clients every six months).
  • Successfully train or teach clients how to develop, grow and practice functional tasks. Examples of functional tasks may include: calling and scheduling appointments; monitoring benefit status/re-certification process; pre-planning for meetings with professionals (medical, legal, probation); meal planning; and completing applications and paperwork (social security, state benefits, LEAP).
  • Work with clients to identify and draw upon natural supports to ensure long-term sustainability of skill development.
  • Be a constant source of encouragement by helping clients to recognize and own their progress and successes. Acknowledge and celebrate effort and accomplishments.
  • Maintain complete and accurate client records in the database.
  • Update case notes using the GIRP model, and include duration, type of contact, summary of activities, and progress at the appropriate time intervals (this includes updating the Goal Attainment Scale (GAS) and Confidence scores at the appropriate time intervals).
  • Networking and Public Awareness/Communications
  • Build capacity within the community through outreach, brain injury education, training, and consultation to professional networks.
