Job summary

Minds Matter LLC is seeking a Physical Therapist Assistant to evaluate, plan and provide therapeutic interventions for individuals with traumatic brain injury. You will work under the supervision of a Physical Therapist in home and community settings, coordinating with case managers to support recovery and independence.

Responsibilities include assessment, program planning, documentation, family education and ongoing communication with the care team.

Qualifications

• High School or GED required; one year+ related experience preferred.
• Associate’s or bachelor’s degree preferred.
• Proficient with Microsoft Office, especially Excel.
• Excellent written and verbal communication skills; detail oriented.

Responsibilities

• Evaluate and plan therapy for individuals with brain injury.
• Provide therapeutic interventions under supervision of PT.
• Maintain treatment records and progress notes.
• Collaborate with case managers to set goals and measure progress.
• Educate families about brain injury and progress.
• Attend trainings to stay current with best practices.
• Schedule supervision meetings bi-monthly or as needed.
• Ensure HIPAA compliance and confidentiality of records.
