Job summary

Vinfen seeks a licensed practical nurse (LPN) in Massachusetts to deliver person-centered care, administer medications, and supervise CNAs within a supportive team. You’ll collaborate with families and external providers to ensure quality care and compliance with medical standards.

Responsibilities include planning and implementing care plans, conducting assessments, coordinating transportation, and maintaining detailed medical records while promoting client dignity and independence.

Qualifications

• Massachusetts LPN license required.
• CPR certification required within two weeks of hire.
• Safety Care certification required within 90 days of hire.
• CBIS certification (preferred).
• One to two years of supervised practical experience helpful.
• Experience with brain injury preferred.
• Valid driver’s license required at employment.
• Must be at least 21 years of age.

Responsibilities

• Develop and implement person-centered treatment care plans.
• Provide daily nursing care and assist with activities of daily living.
• Monitor clients for changes in status and perform assessments.
• Administer medications and treatments as ordered by the PCP.
• Provide transportation and accompany individuals to appointments.
• Ensure safe and timely management of supplies and treatments.
• Utilize behavioral strategies and follow clinical plans.
• Supervise CNAs during the shift and provide nursing education.
• Communicate with leadership, families, agencies, and Vinfen staff.
• Document care, medications, plans, and records.
• Maintain a safe, clean, and regulation-compliant environment.
• Support community outings and respect preferences.
• Advocate for individuals served and collaborate with providers.
• Participate in treatment plan and ISP meetings.
• Maintain relationships with individuals, families, and contacts.
• Comply with accreditation, licensing, fire, safety, and Vinfen policies.
